The Chronic Care Clearinghouse website provides homecare clinicians with the ability to access a single source of information about chronic care management models and best practices designed specifically for homecare patients.
The site utilizes a custom content management system created for the Visiting Nurse Associations of America by Edgewater Technology. I worked with the developers at Edgewater to create the look of the site and acted as an intermediary between the homecare panelists and the developers to make sure of the functionality.
